For your convenience, we have uploaded our patient registration and health history forms so that you can print them, fill them in, and bring them with you to your first appointment. We have also included a records release form, in the event you have been receiving care at another provider's office and would like to have your records transferred to ours. Just fill in your name and birth date and your previous dentist's name, then send it to your previous provider. You can also send it to us and we will contact your provider for you.
